  • JOB SUMMARY/PURPOSE
    • Under the direction of the department chief, nurse leader or designee; provides practice management support to a clinical team, recognizing the patient as the central focus. Provides telephone advice to patients with minor acute problems. Serves as a patient educator in areas related to self-care. Supports the on-going management of patients with stable chronic diseases. Monitors acutely ill patients during the patient visit. Performs a variety of procedures that are within the scope of the professional nursing practice.

    DEPARTMENT DESCRIPTION
    • The Samaritan Family Medicine Residency Clinic (SFMRC) offers complete health care for patients and families across the spectrum of life, from newborns to seniors. The foundation of primary care is to build enduring and continuity focused relationships with patients to promote better health. SFMRC consists of board certified family medicine physicians and resident physicians at various stages of their post graduate training. This clinic provides acute/same day care, chronic disease management, preventative care, women’s health, general pediatric care, office based procedures, and pre/post-natal care that aligns with the clinical competencies required by the Accreditation Council for Graduate Medical Education.

    EXPERIENCE/EDUCATION/QUALIFICATIONS
    • Current unencumbered Oregon RN licensure required.
    • Healthcare Provider BLS required.
    • BSN preferred.

Summary Occasionally JOB SUMMARY/PURPOSE The Registered Nurse (RN) is accountable for utilizing the nursing process to provide safe, age-appropriate, direct and indirect Hospice patient care, while practicing within the scope of practice of the Oregon State Board of Nursing. DEPARTMENT DESCRIPTION The Hospice teams at Samaritan Health Services support patients and family members through the final stages of their illness. The Samaritan Evergreen Hospice House in Albany is a 12 bed inpatient facility that provides symptom management, respite care and room and board. Services offered include skilled nursing care, pain and other symptom management, medical social services, counseling, spiritual support, hospice aides, trained hospice volunteers, bereavement support and medication and equipment as appropriate. EXPERIENCE/EDUCATION/QUALIFICATIONS Current unencumbered Oregon RN Licensure required. Healthcare Provider Level BLS required. BSN and/or previous HH/Hospice or case management experience preferred. Previous broad-based nursing experience to ensure specialized skills needed for HH/Hospice Nursing preferred. AGH Hospice Inpatient: Successful completion of Food Handler's course required within 30 days of hire. Must be able to pass Criminal Records Check upon hire and every three (3) years thereafter.
