Job Summary

Performs procedures which require specialized training for general nursing care and related services for assigned patients, under the direction of a Registered Nurse (RN) or Medical Doctor (MD), in accordance with established policy. Participates in the planning for and provision of patient care. Uses knowledge of asepsis in performance of treatments, such as sterile dressings, irritations and cauterizations. Provides functional direction to ancillary personnel. On call duties will be assigned based on location and agency operational needs. If assigned by agency leadership, incumbent is required to perform on call duties.

Job Requirements

Education and Work Experience:

  • High School Education/GED or equivalent: Preferred
  • Bachelor's Degree or equivalent combination of education/related experience: Preferred

Licenses/Certifications

  • Licensed Vocational Nurse (LVN) licensure in the state of practice: Required
  • Basic Life Support (BLS OR HS-BLS OR RQIBLS) Health Care Provider certification: Required
  • Arrhythmia certification: Preferred
  • Valid Driver's License (DL) and must be at least 21 years of age or older: Preferred
  • Licensed Vocational Nurse/Licensed Practical Nurse (LVN/LPN) or Registered Nurse (RN) or Nurse Practitioner (NP): Required

Essential Functions

  • Provides care for assigned patients, under the supervision of an Registered Nurse (RN) according to established standard and nursing process.
  • Performs routine visits based on departmental specific policy and procedures. Assists in the development of a prioritized interdisciplinary plan of care on each patient.
  • Implements and coordinates plan of care utilizing appropriate organizational skills and reporting mechanisms. Implements individual teaching plans, including Patient Teaching Protocols and discharge Plans, based on mutual goal setting and with emphasis on patient decision making.
  • Performs assigned clinical procedures (i.e. phlebotomy, injections, drug screens, etc.) Administers medications.
  • Orders/restocks supplies as assigned, according to established par levels. Maintains neatness/cleanliness in assigned areas.
  • Performs other job-related duties as assigned.

Communicate information to patients/caregivers, physicians, and providers and assists in making alternative arrangements as dictated by insurance coverage and benefits. • Collaborate and seek input from Clinical Managers when patient’s psychosocial and/or spiritual issues indicate that assessment and interventions may be needed quickly following admission. • Consults with Access Services RNs regarding patient eligibility and complex issues with support from department manager as appropriate. • Maintain positive working relationships with referral sources, outside agencies and co-workers; contribute to the identification strategies to improve agency-responsiveness to referral sources. • Contribute to overall home health/hospice team function through effective time management and communication; utilize critical clinical thinking and problem-solving skills to address team issues. • Responsible for maintaining awareness of professional Scope of Practice. • Adhere to PSCS and other Providence policies and procedures, and applicable regulations. • Participate in Continuous Quality Improvement activities, identify related issues and implement recommended changes as appropriate. Education and/or Experience Required - Degree from an accredited Licensed Practical/Vocational Nurse program. Required - 2 years Nursing experience. Preferred - Prior Hospice, Homecare, Long Term Care, medical/surgical, and/or oncology experience.
