SUMMARY
The Registered Nurse (RN) provides comprehensive skilled care as ordered by the attending physician. The RN educates patients and family members and ensures patient safety. The RN is accountable for completing accurate documentation and remaining compliant with all legal rules and regulations. The RN is responsible for all practices and duties within the scope of practice as outlined by the state.

QUALIFICATIONS AND ATTRIBUTES

  • Registered Nurse with a current license in the state of employment
  • Minimum of one year of RN experience in an acute care setting; home care experience preferred
  • Strong communication skills with patients, families, physicians, and coworkers
  • Ability to remain calm, patient, and compassionate

MAJOR AREAS OF RESPONSIBILITY

  • Provide skilled nursing care in the patient's home as ordered by the physician
  • Observe and monitor patient conditions
  • Perform OASIS assessments to develop and adjust individualized plans of care
  • Administer medications as prescribed
  • Reduce rehospitalizations by prioritizing high-risk patient visits
  • Coordinate multidisciplinary care as appropriate
  • Promote continuity of care through proper admissions, transfers, and discharges
  • Educate patients and families on disease processes and care management
  • Involve patients and families in care plan development
  • Supervise and oversee care provided by aides and LPNs
  • Evaluate patient condition and treatment effectiveness
  • Communicate supervisory findings to care team members
  • Complete clinical documentation in compliance with agency and federal guidelines
  • Document all treatments, assessments, and patient education
  • Maintain an active RN license
  • Communicate schedule changes with appropriate staff
  • Participate in on-call rotations and case conferences
  • Attend required trainings and meetings
  • Maintain a professional appearance
  • Follow all agency policies and procedures

ESSENTIAL FUNCTIONS

  • Travel to patient locations as scheduled
  • Maintain daily attendance at assigned visits
  • Document routine, discharge, and recertification visits within 24 hours
  • Document start of care visits within 48 hours
  • Complete and submit required documentation within company timelines
  • Follow the plan of care within RN scope of practice
  • Provide care within assigned geographic area according to regulations

HEALTH QUALIFICATIONS
Employees must meet all physical requirements of the role. A current negative tuberculosis test or chest X-ray is required at hire, with annual testing thereafter unless prior documentation is provided.

Employees must be free of conditions that would limit their ability to perform job duties. This position may involve exposure to bloodborne pathogens.

Job Summary The Perianesthesia Nurse is a registered nurse responsible for the care of patients being prepared to undergo, or recovering from, anesthesia. The Perianesthesia Nurse is tasked with the medical management of each patient and has the skill and knowledge to address critical medical emergencies pre- and post-surgery. Job Responsibilities Prepare patient for surgery or procedure. Ensure the patient chart is initiated, complete the patient assessment, confirm the procedure and/or surgical site, initiate patient IV, perform ordered labs, dispense ordered preop medications and fully document all information and care provided. Provide instructions to patient and support persons as to what to expect throughout the surgery process. Coordinate patient care needs with other disciplines to ensure continuity of care. Monitor patient recovery from anesthesia and medical procedures. Regularly monitor and record patient vital signs, including heart rate, blood pressure, temperature, and respiration rate. Constantly assess patient for adverse reactions to the anesthesia and respond appropriately. Educate each patient on the potential for reactions or side effects of anesthesia. Provide post-procedure discharge instructions to the patient and authorized patient representative that include physical, psychological, social, safety and educational and needs of the patient. Provide orientation and training of new employees as assigned. Function as testing personnel under CLIA1988 regulations as they relate to point of care testing as required.
