In this role, you will provide comprehensive nursing services, coordinate care with interdisciplinary teams, and support clients through assessment, treatment planning, and health education.

Key Responsibilities:
· Perform comprehensive nursing assessments and evaluate patient health status
· Complete screenings, document medical histories, and record vital signs
· Monitor patient health and coordinate care with treatment team members
· Develop action plans addressing physical and behavioral health needs
· Provide health education, disease prevention, and risk reduction support
· Coordinate medical care, appointments, and response to acute health concerns
· Administer and monitor medications in compliance with regulatory standards
· Educate patients and families on medication adherence and treatment plans
· Monitor lab results, medication side effects, and clinical progress
· Assist physicians and advanced practice providers with exams and procedures
· Coordinate with pharmacies and assist with medication access programs
· Maintain accurate and timely documentation in accordance with policies and regulations
· Respond to medical and behavioral health crises with professionalism
· Support infection control, hygiene, and overall health monitoring efforts
· Collaborate with interdisciplinary teams to ensure continuity of care
· Perform other duties as assigned
Education, Experience, and/or Credential Qualifications:
· Current Registered Nurse (RN) license in the state of practice required
· Willingness to obtain licensure in additional states as needed
· Comfortable providing telehealth services
· For FQHC settings: BCLS certification required; ACLS certification preferred
Additional Qualifications:
· Must have a valid driver’s license with an acceptable driving record and current auto insurance
· Strong clinical judgment and critical thinking skills
· Ability to work effectively with individuals experiencing behavioral health and substance use challenges
· Strong communication and interpersonal skills
· Ability to manage multiple priorities in a fast-paced environment
· Ability to maintain professionalism in crisis situations
· Proficiency with electronic health record (EHR) systems
Physical Requirements
· Ability to exert up to 10 pounds of force occasionally
· Ability to perform repetitive hand, finger, and arm movements for typing and documentation
· Ability to sit for extended periods with occasional walking or standing
· Ability to meet the physical demands of a sedentary work environment
