Job Summary:

The Licensed Practical Nurse (LPN) provides skilled nursing services to patients in their homes, supporting the established plan of care. You will assess, implement, document, and monitor patient care while working closely with the RN and interdisciplinary team. Your role is essential in ensuring patients receive consistent, compassionate, and clinically sound care in the place they call home.

Duties and Responsibilities:

  • Recognize and report changes in patient condition to the RN supervisor
  • Complete timely, accurate clinical documentation
  • Support and carry out the patient’s plan of care alongside the RN
  • Collaborate with therapists and other clinicians to ensure continuity of care

Qualifications:

  • Current Missouri LPN license
  • At least one (1) year of recent LPN experience (acute, long-term care, or home health preferred)
  • Home health experience preferred
