Provide education and emotional support to patients and their families. Facilitate and attend interdisciplinary team meetings to discuss patient care. Document patient care accurately and in a timely manner in accordance with company policies. Valid AZ RN license in the state of practice. Minimum of two years of nursing experience, preferably in hospice or palliative care. Strong assessment and interpersonal skills to effectively communicate with patients and families. Ability to work independently and as part of a collaborative team. Compassionate and empathetic approach towards patient care. Knowledge of hospice regulations and best practices. Basic computer skills for documentation and communication purposes.
