Case Manager
Job Summary:
We are seeking a dedicated and compassionate Case Manager to join our team. As a key member of our care team, you will play a vital role in coordinating comprehensive case management services for our residents. This is a full-time opportunity to make a lasting difference in the lives of our residents and their families.
Key Responsibilities:
Oversee and coordinate care for residents in alignment with individual care plans
Conduct comprehensive assessments and participate in care plan development and reviews
Monitor resident health status, manage medications, and ensure proper documentation to comply with DOH and CMS guidelines
Communicate regularly with families, physicians, and interdisciplinary team members
Provide training and guidance to care staff on clinical procedures and best practices
Support regulatory compliance and quality assurance initiatives
Assist in transitions of care and discharge planning.
Monitor outcomes and re-admissions to support continuous quality improvement
Requirements:
Must be a Registered Nurse
Prior experience of 2 years or more in case management preferred
Strong clinical assessment, critical thinking, and communication skills
