The Direct Support Professional III - LPN is a direct service position providing individual medical support and education to individuals in assigned programs who advocate for all medical needs of assigned program participants focusing on optimal health for all individuals. Hours assigned by supervisor; on call responsibilities.

 

Pay: $40.00/hr.

Responsibilities

  • Assess and coordinate healthcare services for individuals, ensuring timely follow-up and communication with relevant SHCS staff

  • Assist and transport individuals to medical appointments in accordance with DDS standards

  • Document all nursing interventions, medical updates, and care changes in Electronic Health Records (EHR)

  • Maintain and review medication and treatment orders; monitor for side effects and report concerns to appropriate personnel

  • Support Individual Service Plan (ISP) implementation by understanding and monitoring relevant care components (e.g., behavior plans, hygiene, self-medication)

  • Train staff on medication policies and procedures

  • Ensure health records are accurate and compliant with DDS regulations

  • Communicate medical updates and treatment plans to residential and day program staff

  • Maintain safety standards and monitor for compliance across settings

  • Collaborate with Health Care Director or program RN for supervision and clinical guidance

  • Participate in staff meetings and mandatory in-services

  • Uphold agency policies and maintain professionalism, confidentiality, and respect at all times

  • Foster a supportive and cooperative team environment

  • Perform additional duties as assigned by the Health Care Director or RN

Qualifications

Education & Experience:

  • High School Diploma or GED with relevant experience; BA preferred.
  • Licensed Practical Nurse (LPN) or Registered Nurse (RN) with current Massachusetts license.
  • One year working with individuals with Department of Developmental Services (MR/DDS)

Skills and Knowledge:

  • Basic computer literacy; proficiency preferred
  • Valid Driver’s License & Good Driving Record
  • Medication Administration Program (MAP) certification preferred
