Carease Health is seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) with a background in Geriatrics or Internal Medicine to deliver high-quality preventative care to residents in skilled nursing facilities. This hybrid role combines on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation.

Success in this role depends on building strong relationships with the facility, maintaining a patient-centered approach, and being a collaborative team player.

Requirements:

Active NP or PA license (in the state of practice)
1+ year of experience in:
- Geriatric care
- Documentation review
Excellent communication, collaboration, and teamwork skills

Overview The Strelitz Diabetes Center, Endocrine and Metabolic Disorders Division, Department of Medicine is recruiting for an Advanced Practice Provider (APP), a nurse practitioner (NP) or a physician assistant (PA), who will function as a clinical provider. The APP will participate in all aspects of patient care and will participate in the supporting roles of consultant, and educator. The main role of the position will include outpatient care of patients as well as in-patient consulting and conducting hospital inpatient rounds.
