Coler offers long term and subacute care, rehabilitation, and a dedicated memory care unit, comprised of 150 beds featuring an acclaimed music and memory program. The facility features a robust rehabilitation therapy program with expansive gym space for residents. POSITION OVERVIEW: (Primary Purpose of Position)
Oversees all nursing functions in the skilled nursing facility. Works in conjunction with administrative and medical leadership to develop and implement the strategic plan of the Gouverneur. Acts as a change agent, leader, and role model for the nursing profession with the goal of having nurses work at the top of their licenses. Provides guidance and develops action plans to address patient care issues. The Director of Nursing must be a skilled leader able to demonstrate cultural competence in a diverse environment.

ESSENTIAL FUNCTIONS: (List in order of importance to role and time spent on function)
- Plan, organize, direct, coordinate, and evaluate Patient Care Services.
- Demonstrates current clinical knowledge of best practices and regulations impacting the area of practice.
- Develops clinical and administrative policies, procedures, and programs, standards for practice in conjunction with evidence-based best practices and regulatory standards.
- Participate in facility and health care delivery system leadership committees.
- Conduct scheduled rounds at the service levels to identify issues, obtain feedback from staff for improvement strategies, and monitor delivery of patient care.
- Participates in on-site regulatory surveys, i.e., SDOH, CMS, TJC, and development of Corrective Action Plans as needed.
- Conducts monthly meetings with Head Nurses/Associate/ADN and quarterly meetings with the general staff.
- Identifies, addresses, and communicates risk management issues
- Responds to patient complaints and works on resolution in conjunction with service line leadership.
- Identifies and defines problems relating to the delivery of care and recommends solutions or corrective actions.
- Identifies and participates in the development of new care delivery models and programs as applicable.
- Acts as a clinical resource for the department and facility.
- Maintains open communications with all levels of staff and departments in ambulatory care to promote effective working relationships
- Facilitates conflict resolution, ensuring Just Culture and consistent handling of patient, staff, and departmental issues.
- Participates in environmental and infection control rounds.
- Assists in the implementation and monitoring of staff compliance with the facility's Patient Safety Program, as well as Regulatory and Quality activities and projects
- Communicates and engages staff in facility and health care delivery-wide initiatives. Obtains feedback to improve processes and services.
- Assists with the development and updating of the Orientation Program for both professional and non-professional staff.
- Reviews staffing models based on program and external requirements on an annual basis and as needed.
- Formulate/update job descriptions for all Patient Care Service department staff.
- Identify vacancies; interview and process candidates for hire.
- Conducts evaluations for direct reports with plans/goals/opportunities for improvement for the upcoming year.
- Monitors departmental compliance, including but not limited to evaluations, competencies, BCLS, annual mandates, compliance, and others.
- Assist/participate in disciplinary action of employees.
- Identify learning opportunities and develop/schedule in-services/classes in conjunction with OSD, Central Office, DOH, and other agencies as needed.
- Prepares time sheets, verifies attendance, and updates the attendance database.
- Assist in Employee Health Service functions, including but not limited to medical clearances, PPD readings, employee flu vaccine campaigns, and others.
- Identify measures for QA/PI projects for the Nursing department. Collect, analyze, and monitor data, and provide recommendations for improvement and sustainment.
- Participate in service line-specific projects as indicated.
- Participate in facility-wide QA/PI Committee and board report.
- Utilizes Breakthrough methodology as appropriate for developing A3s and projects.
- Conduct chart reviews as needed.
- Act as the facility nursing champion for the Electronic Health Record.
- Identify IT issues/changes to facilitate documentation and workflow.
- Identify hardware/software needs to facilitate workflows for patients and staff.
- Identify IT learning needs and facilitate necessary in-services.
- Participate in the facility-wide HIM Committee.
- Monitor patient complaints in conjunction with the Assistant Director of Assistant Director/Director of Social Work to identify trends and issues.
- Work with departmental leadership to develop strategies to enhance the patient experience.
- Develop work plans to engage staff and meet strategic goals for the Patient Experience Plan for the facility and health care delivery system
- Staff facility-wide initiatives, including but not limited to Open House, flu campaign.
- Participates and performs in all drills: Fire Safety, Disaster/Emergency Preparedness, and Rapid Response Team.
- Attends professional workshops, conferences, and other educational meetings as appropriate.
- Maintains membership in professional organizations to maintain knowledge of current trends, practices, and developments.
- Performs other duties as requested.
- Licensed and currently registered to practice as a Professional Nurse in New York State; and,
- A Master's Degree in Nursing Administration or an equivalent field, and a minimum of four years of high-level responsible experience in nursing administration, three years of which shall have been in a hospital; or,
- A satisfactory equivalent. However, all candidates must be licensed and currently registered to practice as a Professional Nurse in New York State.
- Master’s Degree in Nursing or related field strongly preferred
- Minimum of four years of high-level responsible experience in nursing administration in a Long-Term Care facility; or, a satisfactory equivalent
- Excellent communication skills, both oral and written.
- Knowledgeable of the Nursing Process and the NYS Nurse Practice Act
- Demonstrated leadership ability with strong interpersonal skills.
