JOB DESCRIPTION

HealthPartners North Suburban Family Physicians has an exciting opportunity for a Triage RN to join our team! This .80 fte position will be based at our Hugo clinic with potential for coverage at our Roseville or Lino Lakesclinics.

This position will work in clinic Monday, Tuesday, Thursday, Friday days (start time between 7:45am – 8:30am and end time between 4:15pm – 5:30pm).

This position provides triage support for our clinic at the North Suburban Family Physicians clinics by using protocols embedded in the electronic medical record and a variety of resources including standing orders and protocols. This position also includes occasionally working with patients that have questions or needs once on-site . In addition, this role provides comprehensive assessments to establish and evaluate a plan of care and address changes in a patient’s condition, delegates nursing tasks when appropriate and provides supervision and monitoring of function or activity, creates and implements dynamic teaching plans based upon identified patient need and acts as a resource for other staff.

Required Qualifications:

  • Graduate of an accredited school of nursing
  • Current RN licensure in Minnesota
  • Current RN licensure in Wisconsin (within two months of hire)
  • Current BLS certification for health professionals through the American Heart Association or Red Cross required and maintained. On-line BLS certification is not acceptable.
  • Strongly demonstrated written and oral communication skills

Preferred Qualifications:

  • One-year full-time employment as an RN
  • Triage or Care Management experience within the last three years

  • Uses population management tools such as registries to identify high risk patients and track, monitor and follow-up with their care

  • Informs and supports patients and families to assist them in making informed decisions and navigate the healthcare system.

Education:
