Responsibilities

  • Completing hospice evaluations as requested (admission if comfortable and training would be provided)
  • Conducting in home visits as assigned; assessing the patient’s condition, and teaching the patient and family
  • Developing care plans based on diagnosis and needs
  • Administering medications and treatments as prescribed by physician
  • Communicating patient needs and changes with the team as necessary

Qualifications

  • Must hold a current, valid RN license through the State Board of Nursing
  • Community health/hospice is preferred but willing to train the right candidate.
  • Complies with accepted professional standards and practice.
  • Demonstrates good verbal and written communication, and organization skills.
  • Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
  • Possesses and maintains current CPR certification.
