Essential Functions

  • Provide direct personal care to terminally ill patients

  • Assist with personal hygiene, grooming, and toileting

  • Support safe mobility, including use of walkers and wheelchairs

  • Help with meal preparation, feeding, and light housekeeping

  • Observe and report changes in the patient’s physical or mental condition

  • Offer emotional and psychological support to patients and families

  • Maintain accurate and timely documentation of visits

  • Attend team meetings and participate in Quality Improvement (QI) activities

  • Practiceinfection control and follow safety protocols at all times

About You

Qualifications – What You’ll Bring:

  • Active CNA license in the state of employment (Required)

  • Current CPR certification (Required)

  • Driver’s license and reliable vehicle with insurance (Required)

  • Six (6) months of direct patient care experience (Required)

  • Comfortable working in home settings, hospice facilities, or long-term care environments

  • Demonstrated compassion, patience, and ability to handle emotional situations

  • Willingness to work independently and adapt to changing patient needs

Preferred Background (Not Required):

  • Experience in home health, hospice, palliative care, or oncology

  • Prior roles in hospital nursing, ICU, geriatrics, med-surg, ER, telemetry, or nursing homes

  • Exposure to end-of-life care, wound care, or admissions support

Non-Medical Health Support Helping Hearts provides non-medical personal support. Caregivers must remain within their authorized role and follow the client’s approved care plan. Depending on the assignment, responsibilities may include: Providing medication reminders Assisting with approved blood glucose monitoring Documenting readings when required Observing food and fluid intake Monitoring the client’s physical, emotional and behavioral condition Reporting pain, swelling, bruising, redness or skin concerns Reporting falls, injuries or changes in mobility Reporting confusion or unusual behavior Following established emergency procedures Contacting the Helping Hearts office immediately when concerns arise Completing accurate electronic documentation Caregivers must report concerns rather than independently diagnose or treat them. Caregivers must not assume that someone else has already reported a change in condition. Medication Reminders Medication support is limited to the responsibilities authorized in the client’s approved non-medical care plan. Caregivers may: Provide scheduled reminders. Assist with an approved prefilled medication planner. Place approved medications in a cup when directed. Remind the client to self-administer their medication. Document reminders and client responses. Report refusals, missed doses or other concerns. Caregivers must not independently administer medication, change medication instructions or provide medical advice outside their authorized scope. Transportation When required and authorized, caregivers may: Transport clients to appointments. Assist with approved errands. Transport clients to social activities. Help clients enter and exit the vehicle. Secure mobility equipment appropriately. Follow all client-specific transportation instructions. Maintain a clean, safe and insured vehicle. Report any transportation concern immediately. Caregivers may only transport clients when the assignment has been approved and all transportation requirements have been satisfied. Night and Overnight Shift Expectations Caregivers working night or overnight assignments must remain fully awake, alert and attentive throughout the entire shift. Clock in and out from the approved client location. Remain with the client until the scheduled end of the shift. Remain until properly relieved when continuous care is required. Never leave a client unattended when supervision is required. Never leave early without office authorization. Notify the office if a client or family asks them to arrive late, leave early or change the scheduled hours. Complete all required documentation before the end of the shift. Failure to report for an accepted shift, leaving a client without authorization or failing to follow safety procedures may result in removal from the assignment or additional corrective action. Background and Screening Requirements Applicants must: Complete a comprehensive background check covering up to 15 years, where legally permitted. Complete required abuse, neglect and sex-offender registry checks. Complete a pre-employment drug screening when applicable. Participate in random drug screening when required by company policy. Provide complete and accurate application information. Report criminal charges or legal changes that may affect their ability to provide safe care. Have no disqualifying history that would prevent safe employment, subject to applicable laws and individual review. TITLE: DEPARTMENT: SUPERVISOR: Certified Nursing Technician Nursing Director of Nursing POSITION GOAL: The CNT provides direct patient care including assisting in activities of daily living, bathing, dressing, and feeding of patients, assists the RN / LPN, and reports all changes in the resident's condition to the Charge Nurse. ENVIRONMENT: Humphreys County Nursing Home provides multiple levels of care and services to its residents, through a continuing care plan, in an attractive surrounding while offering an array of personal services and activities. ESSENTIAL FUNCTIONS & RESPONSIBILITIES: 1. Demonstrates Appropriate Knowledge and skills to Coordinate and Organize assigned work Maintains and arranges residents room in a clean and orderly fashion as required • Demonstrates competence in care of specimens and accurately handles specimens at all times. Notifies appropriate staff when resident complains of pain 100% of the time. Complies with policy changes expeditiously and reports any problems 100% of the time. Adapts procedures to meet adult and geriatric residents' needs 100% of the time. Assists in keeping residents dry (i.e. changes gown, clothing, linen when bed becomes wet or soiled. Ensures confidentiality of all non-public information and meets any HIPPA requirements.
