Position Summary
Blue Ridge Care is seeking a compassionate and experienced Registered Nurse (RN) to join our Hospice team. This role is responsible for planning, organizing, and directing hospice care for patients and families, with a strong emphasis on community-based care and patient-centered outcomes.
The ideal candidate is clinically skilled, compassionate, and self-directed, with a strong ability to manage complex patient needs while providing emotional support to patients and families during sensitive times.
Essential Duties and Responsibilities
Complete initial, comprehensive, and ongoing patient and family assessments to determine hospice needs
Provide professional nursing care utilizing the nursing process, including assessment, diagnosis, planning, intervention, and evaluation
Develop and implement individualized care plans focused on palliative care, incorporating patient and caregiver goals
Monitor patient condition and symptom management, revising care plans as necessary
Administer medications and treatments in accordance with physician orders
Educate patients and caregivers on care plans, medications, diet, and self-care techniques
Assist patients with activities of daily living and promote comfort and quality of life
Maintain accurate, timely clinical documentation, including pain and symptom management in the EMR
Communicate changes in patient condition to providers and obtain/document physician orders as needed
Collaborate with interdisciplinary team members to ensure coordinated, holistic care
Support emotional needs of patients and families, promoting dignity and comfort
Participate in interdisciplinary team meetings and care planning discussions
Coordinate medical equipment and additional services required for patient care
Supervise hospice aides and delegate responsibilities appropriately
Participate in on-call rotation and respond to patient needs as assigned
Ensure a safe and supportive environment for patients and families
Engage in ongoing professional development and continuing education
Support quality improvement initiatives and organizational goals
Travel throughout the service area as required.
Education and Experience
Must be licensed or eligible for licensure as a Registered Nurse in Virginia (multi-state compact or Virginia-only license)
Current CPR certification required
Minimum of two years of nursing experience preferred, with at least one year in public health, home care, or hospice
Critical care experience preferred
Strong clinical assessment, critical thinking, and problem-solving skills
Ability to manage high-acuity patients with frequent medication adjustments
Understanding of pain management principles and end-of-life care
Excellent verbal and written communication skills
Ability to work independently with minimal supervision
Demonstrated ability to prioritize and manage workload effectively
Must be able to provide compassionate care and remain composed during crisis situations
Valid driver’s license and reliable, insured transportation required
Physical / Sensory / Cognitive Requirements
Ability to stand, walk, bend, kneel, reach, and lift patients or equipment up to 50 pounds
Ability to remain in physically demanding positions for extended periods
Visual acuity sufficient to assess patient condition and read medical instruments
Hearing ability sufficient to communicate with patients, families, and team members and respond to alarms
Ability to perform essential nursing tasks, including lifting, positioning, and transferring patients
Capability to meet the physical demands of home-based patient care environments
