Department HOME HEALTH - 207151 Worker Sub Type Regular Work Shift Pay Grade Job Description The Home Health Licensed Practical Nurse provides direct patient care in the home setting as a key member of an interdisciplinary team that includes Registered Nurses, Physical Therapists, Occupational Therapists, Speech‑Language Pathologists, Social Workers, and Home Health Technicians. Navigator may also help patients set up appointments for doctor visits and medical tests and get financial, legal, and social support.
