Summary

  • The RN assists the Resource Nurse in the management and care of their assigned patients by means of direct patient care, nursing diagnosis, and patient teaching. Participate in educational programs as an educator/consultant (if applicable).

General Hours of Work

  • 8 or 12 hour shift rotation (weekends, holidays and call rotation)

Education and/or Experience

  • Must be a graduate of an accredited school of nursing. Experience preferred, but not required. Bi-lingual experience preferred but not required.

Certificate, Licenses, Registrations

  • Must have a current registered nurse license to practice in the State of Iowa.  Must obtain and maintain professional certifications prior to working in the applicable areas:
    • Med Surg: ACLS, BLS, PALS
    • ED:  ACLS, PALS, NRP,  TNCC  (within one year)
    • Postpartum: ACLS, PALS 
    • L&D: Fetal Monitoring, STABLE, ACLS, NRP

Essential Functions

  • In collaboration with Resource Nurse makes patient assignments to team members to provide the best and most efficient nursing care.  Logs into Vocera adding room assignments, updates electronic white boards with patients assigned and individual patient information.  Coordinates making post discharge follow-up calls.
  • Performs patient assessments, hourly rounding and initial nursing assessment of all new admissions, admission note as well as on-going assessments and patient notes.
  • Coordinates making physicians rounds and physician notifications with the Resource Nurse.  Carries out new orders and/or communicates new orders to team members to be completed.  Makes necessary observation and suggestions to attending physicians.  Accountable for acknowledging all new orders and following through with them.  Provides bedside report for oncoming staff.
  • Provides bedside care and functions to the patients.  Is responsible for appropriate discharge and patient teaching for their shift and completes discharge routine documentation.
  • Knows the hospital area resource services and insurance certification procedure for patients.
  • Administers medication and treatments per physician orders and reviews H & P, meds, treatments, etc.
  • Utilizes principles of good leadership and management in organizing nursing care on the unit.  Serves as liaison between patient, physician, and staff.
  • Attends and conducts in-services meeting, if needed.
  • Records and documents all clinical and patient data including, but not limited to, narcotic counts, census data, QI information, physician orders, patients charges, and etc.
  • Serves as an instructor for prenatal educational activities and collaborates with clinical leaders to assess, plan, implement and evaluate goals, objectives and content of educational programs (if applicable).
  • Performs other duties and responsibilities as assigned.

Summary The Home Health and Hospice RN is responsible for client care to include direct nursing care, coordination of care, and coordination of services in the home for clients of all ages, unless otherwise specified, in order to assist the client in achieving a maximum level of wellness and independence. General Hours of Work Rotating shifts with weekday/evening & weekend/holiday call involvement. Education and/or Experience Diploma, Associates or Bachelor's degree from a college or university; or one to two years related experience and/or training; or equivalent combination of education and experience. Essential Functions Responsible for coordinating client services, case management, providing assigned clients with direct skilled care, and teaching patient care skills to the client and/or caregiver. Initiates preventative and rehabilitative nursing procedures individualized to the client and according to age specific needs. All duties will be performed within the scope of the Nurse Practice Act. Develops and revises a plan of care based on the initial assessment, continual reassessment of client/family needs, client history, nursing diagnosis, physician orders, expected outcomes, and client goals to include discharge plans when appropriate. Utilize appropriate agency and community resources, be a client/family advocate and follow visit utilization guidelines and protocols. Formulates and implements appropriate teaching plans, communicates significant clinical findings and changes in status to the physician and other involved personnel, including the clients. Thoroughly document all skilled and medically necessary care thoroughly, timely and accurately. The plans of care, the documentation and the care provided, must comply with Medicare, State, Federal, insurance and payer requirements as well as local agency policy. Maintains client and organizational confidentiality at all times. The RN actively participates in the QAPI (Quality Assurance and Performance Improvement) program and completes all required in-service programs to ensure standards are met and skills are kept current. Conducts Home Health Aide supervisory visits, prepares and updates Home Health Aide’s written instructions and documents findings. The Home Health RN must ensure that clients receive appropriate care and all duties and activities delegated to aides are within the scope of the Nurse Practice Act. Collects patient health data. Documents findings utilizing Meditech Home Care and completes appropriate OASIS time point completely and accurately. Identifies expected outcomes/goals individualized to the patient and develops a plan of care that prescribes interventions to attain expected outcomes. Collaborates with the patient, significant others and health care providers in providing patient care. Attends and prepares for active participation in every other week care conferences. Contributes to the professional development of peers, colleagues and others. Decisions and actions on behalf of clients are determined to be in an ethical manner. Considers factors related to safety, effectiveness and cost in planning and delivering patient care. Assists with the intake of referrals in Home care assuring Medicare admission criteria are met and will obtain benefits and preauthorization of services for commercial payers as indicated.
