What You Will Do

As an RN Case Manager, you will provide skilled nursing care in the home while guiding each patient’s plan of care from start to finish.

  • Assess, plan, implement, and evaluate individualized patient care
  • Develop care plans in collaboration with physicians and the interdisciplinary team
  • Complete admissions and ongoing nursing assessments
  • Educate and support patients and families through changing needs
  • Document care accurately and in a timely manner

Salary Range: $72,000 to $90,000 annually, based on experience

The unit includes a 28 bed fully monitored unit. We provide high acuity care to critically ill post-operative cardiothoracic, vascular patients, and medical overflow. Common therapies seen on 4D are chest tubes management, lytic therapy, antidysrhythmic, low dose vasopressors, Bipap and high flow oxygen systems, insulin drips, and precedex. 4D works closely with Neuro Trauma PCU and ICUs for opportunities to cross train between departments. We are part of the Critical Care Cluster, which includes all ICUs and PCUs. 4D offers you the chance to deliver compassionate care in a setting that is busy, team oriented, and fulfilling. Learning opportunities are endless and team members are dedicated to mentoring. Career path programs are available to transition into ICU units with an 18-month track. Qualifications: Education: Graduated from a school of nursing (Associate's Degree, Diploma, or BSN).
