Responsibilities

As a Home Health Registered Nurse, You will:   

  • Assess/monitor physical, emotional, and psychological needs of patients
  • Create home health care plans that align with MD orders and the patient's goals
  • Direct nursing care: administering medications, treatments, and interventions
  • Provide pain and symptom management
  • Educate and support the patient’s family and caregivers
  • Collaborate with an interdisciplinary team
  • Maintain accurate and timely documentation
  • Participate in on-call rotation as required by the local branch

Qualifications

  • Registered Nursing Degree (Associate or Bachelor) from an accredited college of nursing with current unrestricted registration and license in the state of practice and in the state of residence is required  
  • One year nursing practice in a patient care setting required; and home health, geriatrics or other related settings preferred 
  • Valid driver's license, acceptable driving record, and proof of car insurance in accordance with Adoration policy 
  • New nursing graduates may be considered in select markets based on program availability 
  • Current CPR certification

Monitors the provision of care for the residents in his/her assigned unit. Interprets philosophy and objectives of nursing service to residents, families and others. Identifies resident/family learning needs based on the discharge care plan as applicable and carries out appropriate teaching required. Demonstrates ability to coordinate and collaborate with resident/family and other members of the comprehensive health care team. Monitors the administration and accurate recording of medications used. Investigates and follow through on medication errors. Notifies physicians of pertinent observations and reactions, incident and abnormal laboratory and x-ray results as required and documents same promptly. Transcribes physician orders timely and ensure orders are implemented as appropriate. Reports all resident, visitor and work-related accidents/injuries and demonstrates appropriate follow through by investigating, documenting and processing incident reports. Accepts guidance, supervision, and criticism if needed. Practices in compliance with the Nurse Practice Act and regulatory agencies regulations. Assures that a stock level of medications, medical supplies, equipment, etc. are maintained at adequate levels at all times. MDS Coordinator The Green Hills Center for Rehabilitation and Healing is looking for a talented and hard-working MDS Coordinator to join our ever-growing team. We are seeking qualified candidates who have experience as an RN or LPN and are committed to help our patients and facilities receive the support they need. Responsible for completion of the Resident Assessment Instrument in accordance with federal and state regulations and company policy and procedures. Acts as in-house case manager by considering all aspects of the residents care and coordinating services with physicians, families, third party payers and facility staff. Essential Job Functions Oversees accurate and thorough completion of the Minimum Data Set (MDS), Care Area Assessments (CAAs) and Care Plans, in accordance with current federal and state regulations and guidelines that govern the process Acts as an in-house Case Manager demonstrating detailed knowledge of residents health status, critical thinking skills to develop an appropriate care pathway and timely communication of needed information to the resident, family, other health care professionals and third party payers Proactively communicates with Administrator and Director of Nursing to identify regulatory risk, effectiveness of Facility/Community Systems that allow capture of resources provided on the MDS, clinical trends that impacts resident care, and any additional information that has an affect on the clinical and operational outcomes of the Facility/Community Utilizes critical thinking skills and collaborates with therapy staff to select the correct reason for assessment and Assessment Reference Date (ARD).
