Responsibilities:

• Provide high-quality, individualized care to our residents, including administering medications, performing wound care, and assisting with daily living activities

• Develop and implement personalized care plans in collaboration with the interdisciplinary team

• Participate in ongoing assessments and evaluations to ensure resident needs are being met

• Communicate effectively with residents, families, and healthcare team members to ensure seamless care

• Maintain accurate and detailed records of resident care and services

Requirements:

• Current licensure as a Licensed Practical Nurse (LPN) in the state of Wisconsin

• Ability to provide high-quality care with minimal supervision

• Strong communication and interpersonal skills

• Ability to work effectively in a fast-paced environment

• Passion for delivering exceptional patient care and experience working with older adults
