We’re hiring a Director of Patient Care Services – a compassionate and experienced clinical leader who oversees hospice care delivery, supports interdisciplinary teams, and ensures regulatory compliance. What You’ll Be Doing:

  • Serve as a role model and resource for nursing staff and interdisciplinary team members
  • Support staff in problem-solving and determining appropriate clinical interventions
  • Collaborate with the Administrator on hiring, orientation, retention, and performance management
  • Adhere to budget requirements for labor, pharmacy, DME, and medical supplies
  • Coordinate patient care with RN Case Managers and attending physicians
  • Review and manage patients receiving higher levels of care (respite, inpatient, continuous care)
  • Oversee clinical and management responsibilities for assigned teams
  • Serve as liaison between Administrator, clinical staff, patients, families, and community partners

Qualifications:

  • Graduate of an accredited school of nursing with a bachelor’s degree or higher in nursing or a related health field.
  • Current, unencumbered Registered Nurse (RN) license in the state of employment.
  • Active CPR certification required; certification in Hospice and Palliative Nursing preferred.
  • Minimum of 4 years of experience within the last 5 years in hospice, palliative care, supportive care, home health, or a primary care setting.
  • Minimum of 1 year of supervisory experience in a healthcare setting.
  • Experience with Electronic Medical Records (EMR) preferred; familiarity with WellSky is highly desirable.
  • Valid driver’s license with a clean driving record.
  • Reliable transportation and willingness to travel as needed.
  • Ability to lead bereavement support groups and make sound clinical decisions.
  • Flexibility to work outside regular hours based on patient needs.
