Job Summary:

At Carease Health - NY, we are seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) with a background in Geriatrics or Internal Medicine to join our team. As a Nurse Practitioner, you will play a vital role in delivering high-quality preventative care to residents in skilled nursing facilities. This hybrid role combines on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation.

Responsibilities:

Provide preventative care to residents in skilled nursing facilities, focusing on proactive measures for enhanced physical health and well-being
Conduct on-site visits and remote work from home, with support from a dedicated scribe team for efficient documentation
Build strong relationships with facility staff and maintain a patient-centered approach
Collaborate with the clinical team to ensure seamless alignment with facility processes and protocols
Document patient progress and maintain accurate records
Participate in training and professional development opportunities to stay up-to-date with best practices in geriatric care

Requirements:

Active NP or PA license (in the state of practice)
1+ year of experience in Geriatric care and documentation review
