We are seeking a highly adaptable, motivated, and clinically strong Travel Nurse Practitioner to support national clinical operations. This role provides coverage across multiple states to ensure continuity of care during provider transitions, open territories, new market launches, and urgent staffing needs.
When not actively traveling, this provider will support our Advanced Primary Care Management (APCM) program remotely.
This position is ideal for a provider who enjoys flexibility, autonomy, travel, and making an immediate impact in diverse healthcare environments.
What You'll Do
Clinical Coverage
Provide in-person patient care within skilled nursing facilities and post-acute care settings nationwide.
Deliver temporary provider coverage for open territories, provider absences, and urgent staffing needs.
Perform comprehensive patient assessments, evaluations, treatment planning, and follow-up care.
Maintain timely, accurate, and compliant clinical documentation.
Collaborate with facility leadership, nursing staff, providers, and Carease Health teams.
Support continuity of care during provider transitions and facility onboarding.
Advanced Primary Care Management (APCM)
Provide remote APCM services during non-travel periods.
Review patient records and identify opportunities for clinical intervention.
Complete required documentation and follow-up activities.
Participate in interdisciplinary care planning and quality initiatives.
Operations & Growth Support
Support expansion into new markets and facility launches.
Assist with clinical initiatives, pilot programs, and operational projects.
Provide feedback to leadership to improve workflows and patient outcomes.
Build and maintain strong relationships with facility partners.
Travel Expectations
Extensive travel throughout the United States required.
Travel may account for 50–75%+ of working time depending on business needs.
Ability to travel with 24–48 hours’ notice is required.
Overnight and multi-day travel assignments should be expected.
Schedule flexibility is essential.
Carease Health covers approved travel expenses and coordinates travel arrangements.
Qualifications
Required
Active Nurse Practitioner license in good standing.
National board certification as a Nurse Practitioner.
DEA registration (or ability to obtain).
Ability to obtain and maintain licensure, credentialing, and payer enrollment in multiple states.
Strong clinical assessment, decision-making, and documentation skills.
Excellent communication and interpersonal skills.
Ability to work independently and adapt quickly to changing environments.
Preferred
Active licenses in multiple states.
Experience in:
Skilled Nursing Facilities (SNF)
Long-Term Care (LTC)
Geriatrics
Post-Acute Care
Value-Based Care
Experience with APCM, CCM, or population health programs.
Previous travel provider experience.
Experience supporting multi-state healthcare operations.
