About the Role

As a Licensed Practical Nurse (LPN) at Bradford Hills Nursing & Rehabilitation Center, you will:

Summary: The Wellness Nurse plays a key role in health promotion and illness prevention of patient populations through nursing-patient collaboration and patient education. Essential Functions: 1. Promotes wellness and illness prevention using clinical nursing knowledge of disease process, risk factor identification, and illness prevention strategies. 2. Performs all elements of the Medicare AWV with beneficiaries in accordance with Documentation Standards Policy and the AWV Policy and Procedure. 3. Develops a written, age appropriate, individualized prevention plan for each patient including, but not limited to, psychosocial/behavioral risk factors, physiological risk factors, age-appropriate preventative services, recommendations from the USPSTF, and vaccination schedule. 4. Collaborates with physicians and team members to ensure patient receives referrals and follow up care as appropriate. 5. Maintains relevant nursing knowledge of evidenced based strategies related to health promotion and illness prevention, as well as management of chronic conditions. 6. Educates patients and their families regarding disease processes, treatments, risk factor modification strategies, and prevention strategies as appropriate under the nurse’s scope of practice. 7. This position is eligible for up to $15,000.00 Sign on Bonus for those that qualify. ($7,500.00 for those with less than one year of experience) Position Summary: Assist physician with duties necessary to support the needs of the practice working within and to the maximum scope of license Education, License & Cert: LPN ‐ Must be licensed in the state of practice. Experience: One‐year experience in healthcare field preferred but not required Essential Functions: 1. Directs patient to exam room and prepares patient for exam or treatment by identifying height, weight, respirations, blood pressure, temperature, additional testing, and records same on chart. Will also interview patient to obtain history allergies, reconciles medication list and records on chart as requested. 2. Write or call Rx after obtaining surgeon/provider authorization for all department surgeons/providers as needed. Chart as written in patient’s chart 3. Email, fax or call in any insurance referrals.
