  • GENERAL SUMMARY
    • Reports to designated division Director and is responsible for managing designated nursing personnel, coordinating and directing the workflow and functions of the department assigned to facilitate safe and efficient workflow and clinical practices. Has overall responsibility for designated service operations with 24-hour accountability.
  • ESSENTIAL DUTIES AND RESPONSIBILITIES
    • Supervises assigned staff including development of schedules and ensuring that quality patient care is delivered.
    • Resolves patient needs by using multidisciplinary team strategies.
    • Participates in hiring process to include interviewing and selecting applicants, and orienting new staff, conducting performance evaluations and recommending corrective action when needed.
    • Ensures that nursing policies and procedures are implemented.
    • Communicates and interprets policies and procedures to nursing staff.
    • Collaborates with physicians in the proper treatment of patients.
    • Contributes ideas and suggestions to improve methods and productivity within the assigned unit(s) while maintaining high quality patient care.
    • Mentors and coaches nursing staff to ensure continued professional development.
    • Provides emotional and psychological support to patients and their families.
    • Ensures operation of medical and administrative equipment by verifying emergency equipment availability, preventive maintenance and other measures.
    • Ensures nursing supplies are maintained by reviewing usage reports, identifying trends and anticipating needs; submits supply orders as necessary.
    • Seeks guidance from the Director and/or upper management for unusual or unanticipated circumstances that require deviation from financial/operational policies and standards of practice.
    • Maintains documentation of patient care services by auditing patient and department records.
    • Protects patients by carrying out infection-control policies and protocols, enforcing medication administration and storage procedures and controlled substance regulations.
    • Participates in budget development process as directed.
    • Participates in the department’s rotating management-on-call schedule, serving as the designated off-shift leadership representative to support operational oversight and escalation management.
    • Leads, manages, coaches, and trains a team or department, provides guidance, support, and mentors to ensure optimal performance and productivity.
    • Oversees the recruitment, hiring, team/department orientation, performance appraisals and disciplinary actions including up to termination processes for a team or department.
    • Performs other duties as directed.
  • LICENSES AND/OR CERTIFICATIONS
    • Current Virginia state license as a Registered Nurse or Registered Nurse holding a valid Compact State license.
  • MINIMUM EDUCATION AND EXPERIENCE REQUIREMENTS
    • Bachelor’s degree in nursing required. Master’s degree in Nursing or related field preferred.
    • Healthcare/management experience required.
    • Pediatric experience preferred.
    • Possesses excellent leadership, communication, and analytical skills with the ability to plan, establish and maintain a professional work environment.
    • Ability to identify problems and implement solutions for operational and organizational functions.
    • Interpersonal skills necessary to direct subordinates and delegate tasks.
    • Professional knowledge of nursing theory and practice.
    • Technical and professional knowledge unique to the business and/or clinical operations of the designated services (Nursing, Financial, Human Resource, Information Systems, Risk, Governance, Regulations, Planning and Marketing).
    • CPR Training required and/or must be obtained within 45 days of hire.
  • WORKING CONDITIONS
    • Expected to work in a patient care environment with minimal exposure to environmental hazards such as, but not limited to, excessive noise, dust, or extremes in temperatures. Exposure to communicable diseases and moderately adverse working conditions due to the need to perform certain patient care activities can be expected.

Visiting After Hours Nurse (RN) - Per Diem

*Work-from-Home, responding to phone calls triaging care which may include visiting participant's residence when medically necessary* Our Visiting After Hours Registered Nurse (RN) work a rotational on-call after-hours schedule as part of the comfort care and after-hours team. They respond timely to afterhours calls from staff or the participant (or their family) to assess and triage the situation. As licensed clinician they determine the appropriate response or treatment which may include performing a home visit, communicating with the provider on call and/or coordinating care at a facility. The work also includes providing clinical guidance and education to the participant, family and caregivers as needed. Our Visiting After Hours Nurse (RN) works with the In-Home Services and Primary Care team reporting into the Director of After-Hours Nursing and Comfort Care Services team. After Hours - 4:30pm to 8am Salaried position with mileage reimbursement at $.65/mile Provides phone triage and drives to participant’s home/community for direct care as needed. Will support enrolled participants for their designated PACE center From incoming calls supports on-call assessments with participant/family initiating appropriate intervention, including identifying and respond to emergency situations with the ability to handle on-call assessments and triage appropriately. VANs use their discretion to assess symptoms (through a physical or verbal assessment, or a telephonic conversation), analyze potential diagnoses, and based on such diagnoses, determine the appropriate level of intervention (i.e.; education, clinic visit, urgent care, emergency room), recommend a course of treatment, and begin treatment. Demonstrates assessment ability by applying clinical judgment as it relates to the patient population served. Initiates appropriate preventive and rehabilitative nursing procedures and provides treatments and procedures that contribute to the physiological and psychosocial outcomes and stability of the patient. Demonstrates ability to delegate and prioritize tasks to facilitate quality participant care and ensure all participant care needs are met. For example, the VAN is expected to use their discretion to determine call priority by distinguishing between emergent situations, such as end of life crises, and non-emergent clinical needs. Educates and counsels family members on the end-of-life process including the interpretation of changes to a participant’s condition, stages of the dying process, interactions with the participant during the final phase of life, and how family members can manage their emotions during this time.
