Job Summary:
We are seeking a highly skilled and experienced MDS Nurse to join our team at West Haven Healthcare in West Covina, California. As an MDS Nurse, you will be responsible for developing treatment plans for residents that comply with MDS guidelines, ensuring accurate and timely submissions, and maintaining the confidentiality of all resident care information.
Responsibilities:
Conducts and coordinates the development and completion of the Minimum Data Set (MDS) in accordance with current rules, regulations, and guidelines
Maintains MDS schedule and informs social workers of pending MDS assessments
Generates and distributes monthly MDS calendar
Creates new assessments according to MDS schedule
Submits the MDS report in a timely fashion to meet established reporting deadlines
Ensures the accuracy of the MDS report
Ensures that each portion of the assessment is signed and dated by the person completing that portion of the MDS
Creates and transmits any MDS assessments that need correction
Ensures that the MDS report is compliant with current regulations
Keeps abreast of changing MDS requirements and implements changes as necessary
Maintains and periodically updates written policies and procedures that govern the development, use, and implementation of the resident assessment (MDS) and care plan
Informs Administration and other departments affected by changing MDS requirements
Assesses charts and communicates with healthcare teams to create applicable healthcare plans for current and incoming residents
Performs administrative duties as necessary
Coordinates the shipment of facility's services to residing residents
Observes and documents pricing and effectiveness of services
Conducts new resident assessments to help stabilize and improve practices of the facility
Provides OBRA assessments to ensure compliance with Medicaid or Medicare standards
Acts as the main communicator between insurance professionals and facility managers
Works well with other departments to develop good rapport and to assure communication of resident needs and goals
Meets with Director of Nursing regularly to discuss policies and procedures, rules and regulations, standards, department objectives, and philosophy, problems in patient care, MDS, and care planning, etc.
Requirements:
Must possess a Nursing degree from an accredited nursing program
Must be licensed as a Registered Nurse in the State of California
Must possess MDS Certification from an accredited program
MDS experience in a Long-Term care setting
Ability to speak, read, and write English effectively
Emotional maturity, reliability, tact, integrity, and good judgment
Good problem-solving skills with ability to plan, organize, and execute work activities independently
Ability to interact effectively with team members, residents, families, and visitors

Salary: $40 - $46 HOURLY
