Job Summary

We are seeking a dedicated and compassionate Licensed Practical Nurse to join our team at The Avenue at Lyndhurst, a post-hospital rehabilitation and skilled nursing care facility in Lyndhurst, Ohio. As a key member of our healthcare team, you will play a vital role in providing high-quality care to our residents and patients. Responsibilities

  • Provide skilled nursing care and support to residents and patients under the supervision of licensed nurses and physicians

  • Administer medications, treatments, and therapies as prescribed by physicians and according to established protocols

  • Monitor and maintain accurate records of patient progress, medication administration, and treatment plans

  • Develop and implement individualized care plans to meet the unique needs of each resident or patient

  • Participate in interdisciplinary team meetings to discuss patient care and develop effective treatment strategies
