Job Summary:

We are seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) with a background in Geriatrics or Internal Medicine to deliver high-quality preventative care to residents in skilled nursing facilities. This hybrid role combines on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation.

Responsibilities:

  • Build strong relationships with the facility and maintain a patient-centered approach

  • Collaborate with the healthcare team to provide comprehensive care to patients

  • Conduct on-site visits and remote work from home, supported by a dedicated scribe team

  • Develop and implement individualized care plans for patients

  • Perform chronic disease management and monitoring

  • Provide education and support to patients and families

  • Participate in quality improvement initiatives to reduce hospitalizations

Requirements:

  • Active NP or PA license in the state of practice

  • 1+ year of experience in Geriatric care and documentation review

  • Excellent communication, collaboration, and teamwork skills

  • Masters' degree in Nursing or related field
