Job Summary:

At Liberty Center for Rehabilitation and Nursing, we pride ourselves on "Outstanding medical and rehabilitation services". We are seeking a dedicated and compassionate LPN to join our team, working with us to provide exceptional care to our patients. As an LPN 3-11, you will play a vital role in our nursing staff, working a variety of shifts, including days, evenings, and weekends.

Responsibilities:

Provide high-quality patient care under the supervision of the licensed nurse, including administering medications, performing treatments, and maintaining accurate records
Develop and implement individualized patient care plans, in collaboration with the interdisciplinary team
Participate in care conferences and contributes to the development of patient goals and objectives
Maintain a safe and clean environment for patients, visitors, and staff
Communicate effectively with patients, families, and caregivers to ensure seamless care coordination
Perform routine checks on patients, including vital signs, and report any changes or concerns to the licensed nurse

Requirements:

Graduate of an approved LPN program with a valid license to practice as an LPN in the state of Pennsylvania
Minimum of 0 years of experience as an LPN, with a willingness to learn and grow with our team
Strong communication and interpersonal skills, with the ability to work effectively in a fast-paced environment
Ability to lift, transfer, and reposition patients, as needed

Working Conditions:

Work a variety of shifts, including days, evenings, and weekends
Work in a fast-paced, dynamic environment, with frequent interruptions and changes in priorities
Participate in on-call rotation as needed
