RESPONSIBILTIES: Admissions Coordination: Serve as the primary point of contact for new admissions/readmissions, ensuring all aspects of the process are handled professionally and efficiently. Coordinate pre-admission assessments/evaluations with the interdisciplinary team to evaluate potential neighbors' medical and nursing care needs. Communicate effectively with the admissions team and the transferring facility (hospital, community or other care setting). Clinical evaluation: Conduct thorough evaluations of incoming neighbors, including physical, mental, and emotional health evaluations. Review medical histories, medications, and provider’s orders to develop initial care plans. Communicate and document any special needs or considerations for each resident.
