Responsibilities:

Provide high-quality, patient-centered care to our residents, including assessing, planning, implementing, and evaluating individualized care plans
Develop and maintain effective relationships with patients, families, and interdisciplinary teams to ensure seamless care coordination
Collaborate with physicians, therapists, and other healthcare professionals to deliver comprehensive care
Monitor and report patient progress, identifying areas for improvement and implementing changes as needed
Maintains accurate and detailed records of patient care, treatment plans, and medication administration
Participates in quality improvement initiatives to enhance patient outcomes and reduce readmissions
Supports a positive and respectful environment, promoting a culture of empathy, inclusivity, and teamwork

Requirements:

Current licensure as a Registered Nurse in the state of Pennsylvania
1+ years of experience as an RN, preferably in a rehabilitation or long-term care setting
Strong communication and interpersonal skills, with the ability to work effectively in a fast-paced environment
Ability to adapt to changing priorities and circumstances, maintaining a high level of professionalism and poise
