Position Summary:

Nurse Navigator facilitates all stages of the patient referral process.Works in direct coordination and cooperation with referring physicians, specialists, and self-referring patients. The Nurse Navigator is the resource/main contact for patients during the initial consultation phase.

Essential Functions and Responsibilities:
  • Facilitates all stages of patient referral process
  • Maintains operational responsibility for navigation program and development.
  • Ensures proper scheduling of all appointments with specialists

Required:
  • Licensed Registered Nurse with Bachelors Degree in Nursing
  • Oncology Certified Nurse within 18 months of transfer/hire
