Scripps Mercy Hospital San Diego’s campus includes the O’Toole Breast Care Center, a 24-hour emergency room that is a Level I Trauma Center, outpatient physical rehabilitation, minimally invasive robotic surgery and our childbirth unit (including a Level II neonatal intensive care unit).
This is a full time position with 8 hour shifts at Scripps Mercy in Hillcrest.

This role serves as a Structural Heart Nurse Navigator: coordinates patient care from the time of referral to the time of implant, and helps navigate them through the referral and screening process. This includes being the initial contact for patients when referred to our team. The Specialty Nurse Navigator will provide patient education regarding the referral, procedural, and post-procedural process. Additionally, will review patient’s records and help determine initial screening tests, and will be the point of contact for patient phone calls and will assure all screening appointments have been ordered. Will review screening tests' results and enter into TAVR database. Collaborates with TVT registry abstractors to insure data integrity. Will work with TAVR team to improve flow and processes. Works in collaboration with structural NPs and Cardiologists to review data for related projects.

As a Specialty Nurse Navigator, you will be responsible for:
- Formulating custom care plans from assessment and clinical information demonstrating the application of clinical guidelines including standards of care.
- Establishes specific, measurable, attainable, relevant and time specific nursing goals to guide the care planning process and promote self-reliance.
- Assists patients in establishing, modifying, or reinforcing a social support network, thereby promoting self-reliance.
- Identifies and aligns resources that impact high-risk care.
- Coordinates services on behalf of the high-risk patients.
- Work in collaboration with the Healthcare team to track and review data for research projects as well as resource utilization related to relevant specialty and make recommendations as needed.
Required Qualifications
- Current California RN License
- Current American Heart Association BLS and ACLS
- Three (3) years of recent RN experience in a variety of clinical settings
- Knowledge of hemodynamics and experience with advanced mechanical circulatory support; IABP, Impella, Tandem, ECMO, and LVAD
- Bachelor's degree in nursing (BSN)
- Master's degree in nursing
- ECMO Specialist Certificate from ELSO (Completion of ELSO Foundations course)
- Two years of experience managing care of relevant specialized patient population.
