Overview
Receives patient referrals from outside sources and prepares information for admission to homecare. Coordinates the care of assigned patients among a multidisciplinary team to assure quality and cost effective care.

- Demonstrated excellent oral and written communication skills.
- Computer proficient.
- Must be detail oriented and organized.
- Ability to establish priorities, work independently and with a team, and communicate effectively with multiple team members.
- Ability to provide professional, courteous, a positive attitude, and to be respectful of others.
EDUCATION
All post high school education must have been obtained from a recognized College or University.
- Associates Nursing Required
- Bachelors Nursing Preferred
EXPERIENCE
- 3 years Home Healthcare or Acute Care experience Required
LICENSE/CERTIFICATION
- Active Licensed Registered Nurse
- Basic Life Support Certification
