We are currently seeking Nurse Practitioners provide medical services to patients living in long term care facilities.

NP ROLE AND RESPONSIBILITIES:

Provide primary and preventative care for long term care patients residing in skilled nursing facilities.
Manage Residents with acute change in condition.
Advanced care planning and chronic care management.
Prescribe appropriate medications, including controlled substances in accordance with OH Law.
Order, review and/or follow-up with treatments and diagnostic testing.
Communicate with the nursing home staff on a regular and as needed to discuss changes in condition.
and/or the care plan.
Collaborate with primary care physician.

NP QUALIFICATIONS AND EDUCATION REQUIREMENTS

Current, unrestricted NP license in the state of Ohio.
Board Certification.
DEA licensure.
Clinical background in adult, family or geriatric specialties.
Experience working in a long-term care setting.
Ability to work with an EMR.
Ability to manage patients with advanced illness and multiple chronic conditions.
Ability to work within an interdisciplinary care team to coordinate patient care.
Psych experience a plus

Clinical background of wound care; including but not limited to chronic, debilitating, stalled, or infected wounds Must be comfortable with diagnosing and treating diabetic, arterial, venous, and mixed etiology wounds or Kennedy Ulcers. Must be comfortable staging pressure injuries according to the NPIAP Must be familiar with wound care best practice and advanced wound care products; collagens, alginates, foams, silvers, etc. Must have experience with wound vacs and other DME, such as pressure relieving/ reducing devices Strong preference to having wound certification or be open to getting certified Experience working in a long-term care setting. Ability to work with an EMR. Ability to work within an interdisciplinary care team to coordinate patient care. Basic coding knowledge preferred, not required. Ability to plan and care for patient’s identified problem including medication, diet, therapeutic procedures, etc. Ability to consult with other health team members regarding proposed treatment guidelines. Ability to form therapeutic relationships with patients. Demonstrate appropriate communication, interpersonal and organizational skills, and ability to maintain positive relationship with physicians, nurses and other hospital personnel.
