Encore Healthcare is seeking a clinical liaison for our three communities, Encore Wilmington, Encore Foulk, and Encore Parkside
The Clinical Liaison is responsible for clinically assessing patients as part of admissions process regarding eligibility decisions by utilizing on-sight visits, medical records, and professional judgement. The Nurse Clinical Liaison also oversees all marketing efforts by the facility, including developing, generating, and maintaining healthy relationships with any external referral sources.
Essential Duties and Responsibilities:
· Responsible for generating new referrals.
· Developing, generating, and maintaining hospital relationships with CMs/SWs/Physicians.
· Developing, generating, and maintaining community relationships with any applicable referral sources.
· Develop and execute a comprehensive sales and marketing strategy to increase occupancy and revenue over our three communities.
· Takes lead on patient eligibility for Skilled Nursing and Long-Term Care by performing in-person assessments, reviewing medical records, medication lists, and coordinating with facility admissions department.
· Completing written assessment and providing proper documentation to the staff at Encore.
· Responds appropriately and completely to incoming referrals in a timely manner.
· Communicates with patients and their families.
· Generate and complete tours within the facility.
· Interacts with a variety of hospital staff from different departments.
· Coordinates with community staff to ensure that each patient has a smooth transition through the admission process.
· Knowledge of health insurance coverage for patients.
· Ability to obtain insurance authorizations for admission to Skilled Nursing.
· Responsible to know EMR system and admission notification process.
· Schedules speaking engagements at community and professional events.
· Contributes to social media presence.
· Works with all Encore Admissions, Sales, and Marketing teams to bring awareness of the facility’s strengths and services.
· Exhibits professional, positive, and appropriate behavior as a representative of Encore.
· Follows all admissions/marketing policies, structure and established procedures, including communication and documentation tools.
· Attends all necessary meetings within the community.
· Prepares daily and weekly Marketing Reports by tracking referrals and collecting, analyzing, and summarizing data and trends.
· Daily/Weekly conference calls with VP of Business Development.
· Working as a member of the collective admissions/marketing team, including coverage responsibilities for peers in admissions office.

· Must have excellent organizational skills and be an effective time manager and multi-tasker.
Education and Experience:
· Minimum of 1 year experience in a Skilled Nursing healthcare setting.
· Must be able to professionally navigate in a variety of settings such as hospitals, skilled nursing facilities, independent and assisted living communities, private homes, and other settings as needed.
· Must be able to work flexible hours, if needed to accommodate needs of facility.
