East Jefferson General Hospital, part of LCMC Health, is seeking a compassionate, patient-focused Primary Care Nurse Practitioner to join our growing outpatient team. This full-time opportunity is ideal for an advanced practice provider who is passionate about preventive medicine, chronic disease management, and building meaningful relationships with patients across the continuum of care.
As a Primary Care Nurse Practitioner, you will collaborate with physicians and an interdisciplinary care team to deliver high-quality, evidence-based care while helping patients achieve their best health outcomes.

What You'll Do
Deliver Exceptional Patient Care
- Perform comprehensive health assessments, physical examinations, and preventive health screenings
- Diagnose and manage acute and chronic health conditions
- Develop individualized treatment plans and evaluate patient responses to care
- Educate patients and families on disease prevention, treatment options, and healthy lifestyle choices
- Respond promptly to patient calls, messages, and clinical concerns
- Maintain accurate and timely clinical documentation within the electronic medical record
- Recognize and respond appropriately to medical emergencies
- Identify and report signs of abuse, neglect, or other patient safety concerns in accordance with organizational policy
Collaborate & Lead
- Work closely with physicians, nurses, clinical staff, and leadership to ensure coordinated patient care
- Participate in daily huddles, provider meetings, and quality improvement initiatives
- Serve as a clinical resource and leader for the care team
- Contribute to practice operations and support a culture of continuous improvement
Ensure Compliance & Quality
- Adhere to all federal, state, and regulatory requirements
- Maintain patient confidentiality and HIPAA compliance
- Stay current with continuing education and professional development requirements
- Support quality, safety, infection prevention, and performance improvement initiatives
Qualifications
Education
- Master's Degree from an accredited Nurse Practitioner Program required
Licensure & Certifications
- Current Registered Nurse (RN) license issued by the Louisiana State Board of Nursing
- Current Advanced Practice Registered Nurse (APRN) license issued by the Louisiana State Board of Nursing
- Basic Life Support (BLS) certification through the American Heart Association
