About the Role:
As a Regional MDS Coordinator, you will be responsible for coordinating the Minimum Data Set (MDS) process across multiple facilities within our region. This includes ensuring accurate and timely completion of MDS assessments, coordinating with healthcare teams to address quality of care concerns, and maintaining compliance with all relevant regulations and standards. Your strong clinical background and excellent communication skills will enable you to effectively collaborate with staff, physicians, and other healthcare professionals to achieve our goals.
Responsibilities:
Coordinate MDS assessments and updates, ensuring accuracy and compliance with regulations
Collaborate with healthcare teams to identify and address quality of care concerns
Develop and implement strategies to improve clinical outcomes and quality of care
Maintain accurate and up-to-date records of MDS assessments and quality care initiatives
Provide education and training to staff on MDS processes and clinical care standards
Participate in quality improvement initiatives and contribute to the development of quality improvement projects
Requirements:
1 years of experience (education or training in a healthcare setting)
Strong clinical background in a long-term care setting
Excellent communication, organizational, and problem-solving skills
Ability to work collaboratively as part of a healthcare team
Familiarity with MDS processes and regulations (training provided for the right candidate)
