Department Overview Family Medicine at Scappoose is a primary care Family Medicine clinic and Rural Health Center (RHC). The patient population includes pregnant people, babies, children, adolescents, adults, and geriatrics. The patients are generally moderately stable to moderately unstable, with complex medical conditions and SDOH barriers that require medical and nursing interventions. These include, but are not limited to, oral and injectable medications, foot care, telephone and in-clinic triage, care coordination, health education, complex wound care, pain management, and behavioral and/or crisis management. Function/Duties of Position This position will cross-train to support additional primary care clinics within the Department of Family Medicine and the Division of Internal Medicine. Responsibilities include remote phone triage, MyChart message management, results review and follow-up, transitions of care, and other duties aligned with operational needs to support clinic workflows and the RN team. Nurses perform patient visits, telephone triage and advice, and manage an in-basket workload in EPIC where pts. are reviewed for Care Management, Care Coordination, and follow up for hospital and ED discharges.
