• Collaborate with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each resident
• Provide daily care and assistance to patients, including bathing, dressing, grooming, and incontinence care
• Monitor vital signs and report any changes or concerns to the healthcare team
• Assist with medication administration, treatments, and procedures as directed by licensed staff
• Engage with residents through recreational activities, hobbies, and one-on-one interactions
• Maintain accurate and timely documentation of patient care and progress
