As an After-hours Hospice RN you’ll contribute to our success in the following ways:

  • Assesses assigned case load of home health care patients to identify the physical, psychosocial, and environmental needs of patients as evidenced by documentation, clinical records, case conferences, team reports, call-in logs, and on-site evaluations.
  • Makes the initial evaluation and re-evaluates the patient’s nursing needs during each visit.
  • Communicates significant findings, problems, and changes in health conditions, environment, or unsafe facility conditions to the clinical supervisor, the physician and/or other personnel involved with patient care.
  • Counsels, instructs, and implements the plan for patient safety, utilizing patient information, family, and community resources.
  • Participates in the implementation of the plan of care of an assigned case load to ensure quality, continuity of care, and achievement of patient outcomes.
  • Identifies deviations from normal Nursing and normal patient condition status. Initiates appropriate actions based on the deviation.
  • Verifies the Plan of Care prior to each visit and provides care according to physician’s orders, assessment data, and established standards and guidelines.
  • Performs other duties/projects as assigned.

What is Required?

  • Graduate of an accredited Certificate, Diploma, Associates or Baccalaureate School of Nursing.
  • 1+ year of experience as a nurse in a clinical care setting.
  • Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
  • Home Health experience is preferred, but not required.
  • Current State license as a Registered Nurse.
  • Ability to travel approximately 50%
  • Excellent verbal and written communication skills.

You will report to the Branch Manager or Clinical Manager.

UNIT SUMMARY This 72 bed PCU practices patient and family centered care. The unit acuity level of patience is progressive care. Our unit consists of highly motivated, skilled Nurses, Nursing Assistants, and Telemetry Technicians working together as a team. UNIT SUMMARY This Progressive Care Unit (PCU) is a 67‑bed, high‑acuity unit that practices patient‑ and family‑centered care while supporting a diverse adult patient population requiring an elevated level of monitoring and nursing expertise. Our team of highly motivated Registered Nurses, Nursing Assistants, and Telemetry Technicians works collaboratively in a fast‑paced environment that values teamwork, support, and clinical excellence. Nurses on this unit care for a wide range of complex patient populations, allowing them to strengthen advanced assessment and critical‑thinking skills while gaining exposure across multiple specialties.
