About the Role:

As a Home Health Aide (HHA/PCA) with Ideal Home Care, you will be responsible for providing high-quality patient care in the comfort of their own homes. This includes assisting with daily living activities such as bathing, dressing, grooming, feeding, and transferring patients. You will also be responsible for reporting any changes in patient condition or needs to the nursing staff and collaborating with other healthcare professionals to develop personalized care plans.

Responsibilities:

• Provide personalized care and assistance to patients in their homes, focusing on their unique needs and goals
• Assist patients with daily living activities, such as bathing, dressing, grooming, feeding, and transferring
• Monitor patients' conditions and report any changes or concerns to the nursing staff
• Collaborate with other healthcare professionals to develop and implement personalized care plans
• Maintain accurate and detailed records of patient care and services provided
• Perform light housekeeping and laundry duties as needed
• Promote patient independence and well-being, while also respecting their confidentiality and dignity

Requirements:
