Job Description OSF CompleteCare 55+ is an in-person and digital primary care model created to address the needs of adults 55+ who often face multiple chronic conditions, fragmented care, and barriers to timely access. Our clinics are designed to meet the unique health care needs of older adults through a personalized, team-based approach.The Advance Practice Provider with OSF CompleteCare 55+ will support expanding access, and ensuring that patients receive team-based, coordinated care. This may include partnering with leadership, other providers, clinic and virtual care teams, and collaborating with cross-functionally key stakeholders to support consistent, efficient, and standardized processes. OSF CompleteCare 55+ features patient-provider visits in-person or virtually; integration of existing OnCall Digital tools such as Remote Patient Monitoring, Chronic Care Management, Position Highlights

- Provide comprehensive primary care services for patients age 55 and older
- Conduct patient assessments, diagnose and manage acute and chronic conditions
- Develop individualized treatment plans focused on wellness and preventive care
- Collaborate with physicians, specialists, nurses, and care management teams
- Promote healthy aging through education, disease prevention, and lifestyle counseling
- Utilize EPIC electronic medical record system
- Opportunity to develop long-term patient relationships in a value-based care setting
- Manage chronic disease complexities common in the 55+ demographic
- Support extended access strategies (virtual care, remote monitoring, same-day availability)
- Maintain safe provider-to-patient ratios for sustainable panel growth
- Uphold OSF quality metrics, care gap closure, and patient-experience expectations
- Join a growing team of providers in leading the way for embedding technology into primary care.
- Deliver high-quality, safe patient care through a collaborative team model instead of individual patient panel structure.
- Strong clinical abilities for management of a population age ~55+ with multiple chronic health conditions
- Patient care settings will include clinic appointments, virtual visits, group visits, and hybrid options.
- Supports creating a culture of innovation growth, encourages open discussion of ethical and clinical concerns and willingness to give and receive real time feedback.
- Provide consistent, team-based primary care aligned with the CompleteCare 55+ philosophy

- Clinical base hourly pay range for this position: $55.20 - $70.09
- Range shown should be used as an estimate and can be impacted by many factors including but not limited to the critical need of the position, overall experience and qualifications, community need, and other considerations. Additional compensation components may be applicable such as bonuses or incentives which may or may not be based on metric achievement.
Qualifications
Qualifications
- Master's or Doctoral degree from an accredited Nurse Practitioner program or graduate of an accredited Physician Assistant program
- Current Illinois APN or PA license (or eligibility to obtain)
- Board certification required
- Previous primary care, family medicine, internal medicine, geriatrics, or senior-focused care experience required
- Strong communication, collaboration, and patient engagement skills
