As the Hospice Nurse Practitioner, you will provide consultation in palliative care, symptom management and supportive care to meet the needs of the Hospice patient as requested by the referring physician. 

Primary Responsibilities:

  • Makes face-to-face encounter visits within prescribed time limits to qualifying Hospice patients and reports clinical findings to the Hospice Medical Director  
  • Consults with the Hospice Medical Director as needed, informs primary physician of recommended services, and collaborates with other physicians as needed
  • Initiates and coordinates the plan of care. Documents problems, appropriate goals, interventions, and patient/family response to hospice care. Collaborates with the patient/family, attending physician and other members of the IDG in providing patient and family care
  • Instructs and supervises the patient/family in self-care techniques when appropriate
  • Maintains accurate and relevant clinical notes regarding the patient's condition. Reports patient condition and any identified needs/issues to Administrator and Hospice Medical Director

Required Qualifications:

  • Active unrestricted license in the state of residence
  • Current certification as a clinical Nurse Specialist or Nurse Practitioner in the state of practice
  • Current certification by a national body such as ANCC, NBCHPN, or AANP
  • Current CPR certification
  • 5+ years of nursing experience
  • Current driver's license and vehicle insurance, access to a dependable vehicle or public transportation.  

 

Preferred Qualifications:

  • Experience working with an interdisciplinary team
  • Clinical nursing care in oncology, hospice, geriatrics, or intensive care.
  • Palliative Care ARNP

 

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The salary for this role will range from $95,686 to $143,530 annually based on full-time employment. We comply with all minimum wage laws as applicable.

About Our Kids Center: The Our Kids Center specializes in providing forensic medical evaluations for children when there are concerns of child sexual abuse. Serving approximately 750 children in Middle Tennessee each year, Our Kids is a recognized leader in forensic medical evaluations and research in this field. The successful candidate will be a PRN employee at Nashville General Hospital and assigned to the Our Kids Center. Key Responsibilities: · Provide after-hours coverage which includes at a minimum 8 shifts per month, some of which are required to be weekends. [weekday shifts: 5p-8a, weekend/holiday shifts: 8a-8a]. o Triage incoming calls from community partners (DCS, LE, medical providers). o Perform forensic medical evaluations on children and adolescents (birth to 15 years of age) with concerns of acute sexual abuse or assault as part of a multidisciplinary team. · Provide daytime clinic coverage performing non-acute forensic medical evaluations a minimum of 1 weekday per month after training complete. The rapid response team (also known as the Critical Care Outreach Team - CCOT) provides 24 hour coverage across the hospital for episodic care when patients have an acute change in clinical status outside of an ICU. The rapid response/CCOT NP or PA works with the primary team and supervising intensivists to evaluate a patient’s clinical status and identify interventions to stabilize any acute changes while considering moving the patient to a higher level of care. RRT/CCOT is responsible for documentation and data collection related to rapid response and STAT activations as well as education to patients, nurses, and physicians in relation to rapid response and STAT calls ICU Support Team The Critical Care APP works in collaboration with a multidisciplinary health care team. The NP/PA provides healthcare in critical care settings, including surgical and medical ICUs. The APP has didactic education, clinical competency and national certification in identified area of clinical practice. Upon moving to a new ICU, the APP will receive adequate training and support before moving to a more independent role within the ICU for 8 continuous weeks. Each year, the APP will spend blocks of time working on the RRT/CCOT as well as supporting a new ICU for several months where staffing shortages are occurring. The ICU Support Team and Rapid Response team is a dynamic and novel advanced practice provider team who work with physicians across the hospital and the different ICUs to provide care to the most sick and vulnerable patient population. Blocks of several months at a time will be spent staffing either RRT/CCOT or working within a specific ICU. KEY RESPONSIBILITIES Obtains and documents a health history; performs and documents complete, system-focused, or symptom-specific physical examination, assessment and plan of care.
