  • Adheres to the department’s and hospital’s policies and procedures, including safety, risk management and regulatory standards. Keeps abreast of trends, developments and regulatory requirements impacting matters within designated scope of responsibility.
  • Administers, oversees and documents the administration of medications (e.g., topically, orally, intramuscularly, subcutaneously and intravenously) as ordered by the healthcare practitioner and in accordance with hospital policy.
  • Assesses patient to include physical, psychosocial, cultural, behavioral/emotional, developmental and spiritual needs. Gathers data relevant for patient care. Implements appropriate patient interventions and evaluates outcomes. Plans care appropriately based on needs identified during initial and on-going assessments. Provides holistic, comprehensive, and professional nursing care to patient within the scope of practice. Implements appropriate patient interventions and evaluates outcomes. Prepares patient for discharge and assists with coordinating post discharge needs.
  • Cultural Sensitivity and Competence: Demonstrates proper use of available communication tools/materials for effective communication and understands how the culture(s) of patient populations can affect communication, collaboration and the provision of care, treatment and services. Patient Populations Served: Demonstrates knowledge and proper skills associated with the department’s defined specific populations served.
  • Demonstrates competent professional practice and engages in opportunities for professional growth. Demonstrates self accountability to meet stated deadlines for all mandatory requirements.
  • Documents care delivered and patient status utilizing the nursing process in accordance with FHS standards of care, for example, changes in patient condition, patient responses to intervention and reassessments of pertinent findings. Receives and assures appropriateness, accuracy, timeliness and execution of physician orders.

  • Minimum 6 months previous RN experience
  • Current American Heart Association Healthcare Provider BLS certification. Please be advised that this location requires quarterly American Heart Association (AHA) Resuscitation Quality Improvement (RQI) competency certification post-hire.  This ongoing competency is required through resources internal to the facility; outside RQI competency resources are not acceptable for CommonSpirit Health compliance requirements.

JOB DESCRIPTION SUMMARY The registered nurse plans, organizes and directs hospice care and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES Patient Care 1. Completes an initial, comprehensive and ongoing comprehensive assessment of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness(es). 2. Provides professional nursing care by utilizing all elements of nursing process. 3. Assesses and evaluates patient’s status by: A. Writing and initiating plan of care B. Regularly re-evaluating patient and family/caregiver needs C. Participating in revising the plan of care as necessary 4. Initiates the plan of care and makes necessary revisions as patient status and needs change. 5. Uses health assessment data to determine nursing diagnosis. 6. Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process. 7. Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care. 8. Counsels the patient and family in meeting nursing and related needs. 9. Supervises ancillary personnel and delegates responsibilities if required. 4. Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-service classes. 5. Fulfills the obligation of requested and/or accepted case assignments. 6. Actively participates in quality assessment performance improvement teams and activities. POSITION QUALIFICATIONS 1. Graduate of an accredited school of nursing 2. Registered Nurse, with a License in good standing to practice in the state of Virginia. 3. Registered nurses shall have a minimum of (a) one (1) year of experience as a professional nurse within the last three (3) years; OR have a baccalaureate degree in nursing and minimum of two (2) years’ experience, at least one of which is in the area of public health, home care, or hospice nursing is preferred. 4. ALTERNATING WEEKENDS Job Summary: Provides comprehensive skilled nursing care and services to patients in the home setting.
