The Paramount at Somers Rehabilitation and Nursing Center is seeking compassionate and dedicated Home Health Aides (HHAs) to join our team. As an HHA, you will play an essential role in enhancing the quality of life of our residents by providing personal care, support, and companionship in a warm and resident-centered environment.

Responsibilities:

• Assist residents with activities of daily living, including bathing, dressing, grooming, and toileting.
• Help residents with mobility, transfers, and ambulation.
• Assist with feeding and hydration as needed.
• Observe and report changes in residents' conditions to the nursing team.
• Provide emotional support and companionship to residents.
• Maintain a clean, safe, and comfortable environment for residents.
• Follow care plans and facility policies and procedures.
• Work collaboratively with nurses and other healthcare professionals to ensure exceptional resident care.

Qualifications:

• Current Home Health Aide (HHA) certification in the State of New York required.
• Previous experience in long-term care, rehabilitation, or a healthcare setting preferred.
• Compassionate, dependable, and patient-centered approach to care.
• Excellent communication and interpersonal skills.
• Ability to work effectively as part of a team.
