Intake Licensed Practical Nurse (LPN) or Registered Nurse (RN) - Per Diem

This candidate must reside in New Hampshire.

This is a per diem, hybrid position with work performed remotely (in NH) and/or in the New London office. The ideal candidate is self-motivated, tech-savvy and an excellent communicator.

We are looking for a team player who is willing to jump in and help, is flexible, able to prioritize and stay organized. A detail oriented Licensed Practical Nurse (LPN) or Registered Nurse (RN) to join our Intake Department as a per diem teammate. This person will assist with both Homecare and Hospice referrals.

Responsibilities for Intake Licensed Practical Nurse (LPN) or Registered Nurse (RN) - Per Diem:

  • Accurately enter patient referral information into the electronic medical record system
  • Accurately enter all incoming patient care orders and notify appropriate team members as necessary
  • Navigate multiple electronic referral software portals
  • Communicate with discharge planners, physician offices and liaisons regarding patient referrals while maintaining positive relationships
  • Triage all incoming phone calls
  • Communicate effectively with patients and patient care givers

Requirements for Intake Licensed Practical Nurse (LPN) or Registered Nurse (RN) - Per Diem:

  • Active NH Licensed Practical Nurse (LPN) or Active NH Registered Nurse (RN) License
  • Must reside in New Hampshire
  • 2 years of Health Care experience
  • Knowledge of medical terminology, familiarity with a medical office setting, and/or triage
  • Knowledge of Home Health/Hospice
  • Must be comfortable working in a remote environment
  • Proof of Flu vaccination
  • Reliable transportation

  • Physical exam within the last year
