Kendal at Hanover is a non-profit Continuing Care Retirement Community and home to 400 residents.
Kendal at Hanover is seeking a Care Manager - Memory Care (RN or LPN) to provide leadership and oversight for resident care coordination, documentation quality, and care plan development with a primary focus on the Assisted Living Level 1 and Memory Care Neighborhoods. This role integrates clinical supervision and care management with a focus on compliance, interdisciplinary coordination, and high-quality resident-centered care.
Primary duties include care planning, conducting care conferences, order entry, clinical triage, appointment coordination, family communication, and documentation audits. As one of the Nurse Supervisors on duty, this role also provides supervision and support to nursing staff and may assume direct nursing care responsibilities including medication administration and wound care.
Education and Experience Requirements
In order to be considered for this role, an individual must:
- Hold current LPN or RN licensure in the State of New Hampshire and be in good standing without provisions;
- Have a minimum of two (2) years of clinical nursing experience with a preference for experience in post-acute care settings, geriatrics, or care management;
- Have demonstrated knowledge of appropriate skills for communicating with all ages, especially the geriatric population
- Be able to walk, stand, and bend most of the shift, and be able to move up to 50 pounds.
The schedule for this position is Monday through Friday from 8:00 - 4:30 PM.
Salary range for LPN: $40.00 to 49.00 per hour, dependent on experience
Salary range for RN: $44.50 to $55.00 per hour, dependent on experience
