The Dane County Department of Human Services (DCDHS) - Badger Prairie Health Care Center (BPHCC) is currently recruiting for Registered Nurses (RNs). BPHCC currently has has one 0.6 FTE and several 0.2 FTE RN positions.

DEFINITION
This position is located in the Badger Prairie Health Care Center (BPHCC). Under general supervision of the Director of Nursing Services or Nurse Manager, incumbents in this classification perform skilled nursing services within the scope of the Registered Nurse License. Coordinate and provide resident care on the households/neighborhoods. Provide leadership and direction to nursing staff. Collaborate with inter-disciplinary teams. Perform other related work as required.
 
EXAMPLES OF DUTIES
Identifies resident care needs and develops and implements the plan of care for residents. Records resident information in the electronic medical record. Prepares and administers medication to residents. Performs prescribed nursing treatments. Completes resident assessments. Observes and records resident’s response to medication, physical and mental condition. Coordinates the duties and responsibilities for the shift with the charge nurse, Clinical Care Coordinator and/or the Nurse Manager. Communicates and coordinates implementation of the care plan. Coordinates and collaborates with other members of the interdisciplinary team to facilitate and evaluate resident care. Monitors CNA performance of duties for completeness and adherence with resident’s plan of care. Maintains the confidentiality of all resident personal health information and complies with all HIPAA requirements. Assumes charge nurse functions, including scheduling. May care for residents who have infectious pathogens.Education, Training and Experience:  
Applicants must possess an Associate’s or Bachelor’s degree in Nursing.
 
Certifications/Licenses: 
Must be licensed as a Registered Nurse and legally able to practice as such in the State of Wisconsin. Must also be certified in Cardiopulmonary Resuscitation (CPR).
 
Special Requirements: 
Must have a valid driver’s license, or access to reliable personal transportation, or equivalent. Access to a telephone. No previous criminal convictions for abuse, mistreatment or neglect of others. Free of clinically apparent communicable disease. (Refer to other special requirements noted in the ‘Physical and Environmental Work Requirements’ section below.)

Physical and Environmental Work Requirements:
Ability to operate equipment and machinery requiring complex and rapid adjustments such as sphygmomanometer, otoscope, syringes, feeding pumps, IVs, O2 cannulas and concentrators, nasal tubes, stethoscopes, glucometer, nebulizer, suction machines, and oxygen equipment, and thermometers. Ability to handle, load and unload, and move and guide material using simple tools. Ability to exert physical effort in moderate to heavy work, including lifting, carrying, pushing and pulling, and moving 50 pounds unassisted. Ability to stand for extended periods of time. Ability to cope with the mental and emotional stress of the position and be in good general health. Ability to relate to and support individuals within the facility who are mentally/physically disabled, elderly, emotionally upset, and at times hostile. Ability to run at least one (1) flight of stairs to respond to emergency situations. Ability to assist in the evacuation of residents. Potential for exposure to infectious waste, diseases, conditions, etc., including blood borne and air borne pathogens. May be required to wear personal protective equipment, up to and including, an N95 respirator.Knowledge of terminology, theory techniques and current standards of practice. Knowledge of medication, usage, effects, adverse reactions and contraindications. Ability to collaborate in development and implementation of a resident care. Ability to analyze data and information using established criteria. Ability to work collaboratively and cooperative with inter-disciplinary team and lead, assist and monitor the work of nursing personnel. Ability to communicate effectively with inter-disciplinary teams and families/guardians. Must be fluent in speaking and writing the English language in an understandable manner.

The MDS Coordinator (RN) is responsible for overseeing and managing the Resident Assessment Instrument (RAI) process, ensuring accurate and timely completion of Minimum Data Set (MDS) assessments. Key Responsibilities MDS & RAI Process Complete, coordinate, and submit all MDS assessments (OBRA and PPS/PDPM) accurately and within required timelines. Ensure compliance with federal and state regulations, RAI guidelines, and facility policies. Conduct interviews with residents as required for MDS sections (e.g., mood, cognition, preferences). Review clinical documentation for accuracy and ensure supporting documentation is present for all coded items. Compliance & Quality Measures Track and monitor Quality Measures (QMs) and assist with improvement strategies. Maintain up-to-date knowledge of RAI guidelines, CMS regulations, and PDPM updates.
