Duties: Role model for the company’s Vision Statement and collaborates with the Administrator to ensure implementation and compliance. Drives ongoing QAA process, oversees milestone plans for performance improvement, and monitors clinical trends and outcomes. Uses TSI reports, CMS reports, current survey status and clinical outcomes to monitor clinical progress monthly. Communicates care delivery trends, issues and opportunities to administration, medical director, and the Quality Assurance Committee. Anticipates customers’ needs and works to minimize potential problems. Credentials: Currently licensed as RN in this state; Bachelor’s Degree in Nursing preferred Five years in long-term or acute health care preferred; At least 2 years nursing supervisory experience required. Must have, as a minimum of 6 months experience in rehabilitative and restorative nursing practices.
Wound Care Nurse (RN and/or LPN) POSITION SUMMARY: The Wound Care Nurse (RN or LPN) reports to and receives general direction from the Director of Nursing and is responsible for delivering quality wound care and preventative measures to maintain skin integrity to assigned residents in compliance with applicable federal and state guidelines and established policies and procedures. ESSENTIAL JOB FUNCTIONS: • Utilizes the nursing process by demonstrating appropriate assessment, planning, implementation and evaluation of residents wound and skin integrity • Collecting and documentation of residents’ skin status including medical history. • Wound care prevention and prevention through plan of care • Sets priorities for nursing action for wound care and prevention • Communicates with physicians regarding resident wound needs • Initiates nutritional interventions to ensure that residents with wounds are provided nutrition to promote wound healing. • Performs direct resident care utilizing established procedure, policies, and standards • Administer treatments and procedures in accordance with the physician’s orders • Maintains the comfort, privacy, and dignity of each residents while performing treatments • Communicates and interacts effectively, courteously, and tactfully with residents, visitors, family members, peers, and supervisors • Conducts rounds frequently to ensure that residents with incontinence are receiving pericare and preventative ointments timely.
