Accurately documents all patient care per policy and payer requirements; communicates timely with physicians, patients, families, and agency staff to ensure coordinated, effective care. Participates in orientation and precepting of new staff; contributes to performance improvement, survey readiness, patient safety initiatives, and reducing avoidable hospitalizations. Demonstrates professionalism through punctuality, positive attitude, ongoing learning, meeting deadlines, active meeting participation, adherence to agency standards, and participation in on‑call/weekend rotations.
